Your reply seems to answer a) as your conf does not contain INDEX=MEMORY,
so I guess there are current index files. Can you verify there are?
What about b) and c)?
Do you have log entries like "UID inserted in the middle", "Invalid cache"
or something? Is the mail storage located locally or remote / NFS / ... ?
