This one works for me for mysql in a non-standard location (my.cnf is in 
/etc):

CPPFLAGS='-I/opt/mysql/include/mysql' LDFLAGS='-L/opt/mysql/lib/mysql 
-lmysqlclient -lz -lcrypt -lnsl -lm' ./configure --with-mysql .....
