here, my cache _were_ all turned off, and offline was off, too.
in the end, per your comments, I've settled on
browser.cache.memory.capacity" 40000000 browser.cache.memory.enable" true browser.cache.memory.max_entry_size 40000000 mail.imap.mime_parts_on_demand" false mail.server.default.mime_parts_on_demand false
easily deployed to user.js to prevent this for others.
