0.99.x's mbox code is pretty bad. Try 1.0-stables or 1.0-tests instead and set mbox_very_dirty_syncs=yes. That should make Dovecot pretty fast as long as you don't delete old messages.
Thanks for your consideration. However, I'm running maildir, and I think something's fishy with MacOS's directory handling. Haven't figured out what, though.
