I have the following setup:
default_mail_env = maildir:/var/maildir/%u/:INBOX=/var/mail/inboxes/%u/:INDEX=/var/mail/indexes/%u/index
/var/maildir is a nfs mounted directory on a filer /var/mail is local I keep the index files in a separate place because I don't want them to be in the backup.

These should be pretty rare nowadays, so I guess they're stored in NFS? Have you looked at http://wiki.dovecot.org/NFS
