Hmm, your question deals with authentification, but your "relevant parts" omit any instance of passwd. This seems to be contratictious.
You could try to trace the auth processes, when it can no longer authentificate users, see: http://wiki.dovecot.org/Debugging/ProcessTracing Do you see an active mySQL connection?
