In my setup, the index is built externally and will _always_ be
up-to-date.  Getting the last uid would mean running a query, which,
whilst inexpensive, seems unnecessary if my understanding of the
purpose of get_last_uid() is correct.

I guess I left out the most important question - what could I return as
last uid to indicate 'all indexed'?
