How do I setup Dovecot to read both local UNIX account maildirs and a separate virtual user area? Or do I need to run two servers?

With 0.99 you need two servers. With 1.0-tests you'd define two auth sections (http://wiki.dovecot.org/moin.cgi/MultipleAuth).

We run a script on our server that constructs a passwd-format file from the /etc/passwd entries and our virtual users information. Dovecot can then use this file for authorising all our users. Voila.
