I think it's something more. Apparently it's not even looking at the database. Not sure what I didn't do. If anyone can point me to a good dovecot / mysql quota how to that would be helpful.

Check if new quota is recognized using 'doveadm quota get -u <username>'
If it does then there is some other issue... It is hard to tell without knowing how it is setup.
