Yes, considered and rejected all of them so far. I wrote about this a

Although I think bug tracking and feature tracking are quite
different things. I don't think feature tracking needs many features.
I'm pretty happy with the current static html page. Although I did
consider adding it to wiki, because it would provide change history.
Maybe I'll still move it there.

Regarding your three issues:
Yes, they all suck in different ways. You have to pick one with the least suck for you. ;)

What if you setup bug mails to come to this list?

If Timo's not using PHP or MySQL for anything other than bug-tracking, those just become two more things to have to configure and maintain. Where/how they're hosted just changes how much setup and maintenance need to be done. It can't be eliminated fully.

Isn't that what dovecot-bugs@dovecot.org does? Or was the original suggestion never implemented?

I'd want two things from BTS related to this:
It needs to be bidirectional: anyone on this mailing list must be able to reply to the mail from BTS and the reply must be added to BTS. What BTSes support this?
It should be easy to create new bugs from existing mails on mailing list. It would also be nice if replies to the original mail were added as comments to the BTS as well. This gets tricky to implement well.

The only thing I can speak to personally is Debian's bug tracker, which supports this.
