This has nothing to do with the backend and everything to do with the 
Local Delivery Agent.  Whether you are using a database backend or a 
filesystem backend, the LDA would have to be able to query an index for 
your functionality to be possible.  I can easily write a script to 
update local user configuration for for a Maildrop, procmail, or SIEVE 
filter to do exactly what you want, without relying on a database at all.
