Are you using filesystem quota? If so, the quota plugin itself doesn't
really do anything. Do you see any errors in Dovecot's log files? If
not, see the "Logging" section in http://wiki.dovecot.org/LDA

Your problem is most likely that Dovecot couldn't handle the
out-of-quota condition in its index file handling, or something like
that. See http://wiki.dovecot.org/Quota/FS
