Sounds a little like what can be implemented with Signals on DBus.

Dovecot supports plug-in modules.  As listed on the documentation page there's 
already quota, ACLs, trash, convert (converts mailbox format on login), zlib 
(access gzipped mboxs), dspam, and others.

Would being able to 'wrap' a command handler satisfy your needs?
