[Dovecot] "Disconnected in APPEND" sometimes when with attachment

It means that client was saving the message with APPEND command, but
then the connection got disconnected. There's nothing you can do about
it on Dovecot's side. Potential reasons:

1. broken software firewall / antivirus running on client computer
2. something broken/misconfigured in network
3. client bug
