Did you configured mailertable to deliver the messages through the 
"dovecot" mailer?

I prefer:

Won't work with deliver, maybe only if it is SUID.

If Dovecot is the only process accessing /var/mail/file, you could:

a) use another location for INBOX,
b) use flock.
