See the "y=proxy_maybe". It should be "proxy_maybe=y", meaning you
should set:

pass_attrs = uid=user,userPassword=password,=proxy_maybe=y,mailHost=host

(Some day I need to redesign this LDAP configuration. It's horribly
ugly.)
