Thunderbird doesn't support DIGEST-MD5 as far as I know, but mutt has
always used DIGEST-MD5 with me automatically. In any case there isn't
anything on Dovecot's side you can do about this. Although you could
verify that:

openssl s_client -connect imap.domain.org:22993
x capability

There should be no AUTH=PLAIN in the capability list, and there should
also be LOGINDISABLED capability.
