Things get re-indexed on client request if Dovecot sees that index is
stale. So you are buying faster response times for clients with somewhat
higher server load at mail delivery.
I don't know how this piecemeal update of the index stacks up against a
complete re-index, but I'd assume it to be more efficient (only having
to do new mails instead of whole mailbox). Still, I find the re-index to
be almost instantaneous on not-so-small mailboxes (hundreds of MB) and
fairly modest hardware, by today's standards.
